Your Certified Case Manager will save valuable time by addressing issues like safety concerns, multiple disease management, and physical and cognitive decline.  The staff at Independent Opportunities values the importance of mental stimulation, social connections, emotional well being, and spiritual contentment.  Case Managers address individual needs to strive for the best possible quality of life.

We assign a Case Manager who will coordinate an individual treatment plan for each client.  Dependent on the needs of the individual, services will them be acquired from other ADvantage contractors.
